Diseases of the Circulatory System
ICD-10 Code: I25.5
Ischemic Cardiomyopathy
Cardiomyopathy caused by coronary artery disease, typically with reduced ejection fraction.
Clinical Information
Ischemic cardiomyopathy: LV dysfunction due to CAD. Most common cause of systolic HF in US. Diagnosed by CAD on cath + reduced EF on echo. Distinct from non-ischemic cardiomyopathy (I42.x).
Coding Guidelines
- 1Use when cardiomyopathy explicitly attributed to CAD/ischemia
- 2Implies both CAD and LV dysfunction
- 3Often coexists with HF codes
Common Uses
- Cardiology management of CAD with reduced EF
- Heart failure due to ischemic disease
Related ICD-10 Codes
I25.10Atherosclerotic heart disease
I50.22Chronic systolic heart failure
Documentation Requirements
- Document 'ischemic cardiomyopathy' or 'CAD with cardiomyopathy'
- EF percentage
- CAD history
Real-World Coding Examples
62-year-old with ischemic cardiomyopathy (EF 30%), post-MI x 2. On GDMT. Diagnosis: Ischemic cardiomyopathy (I25.5), Chronic systolic heart failure (I50.22)
